What is methylphenidate oral solution?

You, or your child, have been prescribed a medicine called methylphenidate oral solution, to help treat ADHD symptoms. This medicine is taken as part of a wider treatment programme, which may include things like psychological counselling and educational support.

 

Methylphenidate belongs to a group of medicines called stimulants. These work by increasing activity in the brain, particularly in areas that have a role in controlling attention and behaviour. This can help children with ADHD to channel hyperactivity, be less impulsive and concentrate better.

 

Methylphenidate oral solution works in the same way as immediate release methylphenidate tablets. The only difference is that methylphenidate oral solution is a liquid.

 

How is methylphenidate oral solution being used as part of ADHD treatment?

Your doctor will have prescribed methylphenidate oral solution for one of the following reasons:

 

Starting treatment
Methylphenidate oral solution may be used when first starting stimulant therapy, with the dose gradually increased by a doctor until the right dose to meet individual needs is found. This is called dose titration.

 

Switching treatment
You or your child may have already had dose titration with tablets, but have been switched to methylphenidate oral solution because it meets your/their needs better.

 

Needing top-up treatment
You or your child may be taking extended release methylphenidate tablets, but need a ‘top-up’ with an immediate release formulation of methylphenidate. This can help with after school activities or homework in the evening, for example, when the effect of extended release methylphenidate starts to wear off later in the day.

How is methylphenidate oral solution given?

Methylphenidate oral solution comes in a bottle with an accompanying graduated syringe. The syringe is marked in 0.5 ml increments.

 

You simply draw up the volume of methylphenidate oral solution prescribed by the doctor, place the syringe inside the mouth – against the cheek – and press the plunger slowly to gently release the medicine. Then swallow the medicine with a drink of water. It has a neutral to sweet taste, with no artificial flavouring agents or sweeteners.

 

You can give methylphenidate oral solution on an empty stomach, with food or after eating. Giving this medicine with food may help prevent stomach pains, feeling sick or being sick.

Understanding ADHD

If you have ADHD, or are a parent or caregiver of a child with ADHD, you are certainly not alone. That’s because ADHD has been estimated to affect 1 in 20 children. There are many children with ADHD, as well as a team of healthcare professionals treating ADHD in children. Never be shy of sharing concerns, as help is always at hand.

 

Cause of ADHD
ADHD is a medical condition caused by differences in the development, structure and function of the brain, compared to those who don’t have ADHD.

It’s these differences that can cause children with ADHD to struggle with things like: Hyperactivity, concentration, impulse control.

Happily, proven treatments are available, to help children with ADHD at home, during social activities and at school.

While children with ADHD can face many challenges, the condition needn’t affect the ability to succeed. In fact, ADHD can be associated with a range of things that can be huge positives in life, such as:

Energy – the ability to work hard, play hard and strive for success

Creativity – looking at things differently, with a lively mind that’s brimming with ideas and unrestrained by the shackles of convention

Flexibility – never being stuck in a rut, with the ability to consider several options at the same time, chopping and changing as needed in order to solve problems and get results

 

Combining medicines such as methylphenidate oral solution with behavioural therapy and educational support can help children with ADHD to make the most of their talents and abilities, in whichever areas of life they choose.

Giving your child MPH oral solution

Step-by-step instructions
Methylphenidate oral solution comes in a bottle with an accompanying graduated syringe. The syringe is marked in 0.5 ml increments. Here’s how to give a dose of methylphenidate oral solution to your child:

  • Shake the bottle for at least 10 seconds and remove the cap
  • There is a plug with a hole in the neck of the bottle – insert the end of the syringe into the hole and ensure it is secure
  • Hold the syringe in place, turn the bottle upside down and gently pull down the plunger to the required dose. For example, if the volume prescribed is 5 ml, pull down the plunger until the methylphenidate oral solution is on the 5 ml mark on the syringe
  • Turn the bottle the right way up, then remove the syringe by holding onto the bottle and twisting out gently
  • Place the syringe inside your child’s mouth (against their cheek) and press the plunger slowly to gently release the medicine
  • Have your child swallow the medicine with a drink of water
  • After use, the plug should remain in the neck of the bottle
  • For hygiene reasons, wipe the plug and neck clean and rinse the cap before replacing and wash and dry the syringe by hand without removing the plunger

You can give methylphenidate oral solution on an empty stomach, with food or after eating. Giving this medicine with food may help to stop your child from having stomach pains, feeling sick or being sick.

IMPORTANT
Methylphenidate oral solution should always be given exactly as prescribed by your child’s doctor, in terms of both volume administered and frequency of administration.

 

If your child misses a dose, do not give a double dose to make up for it. Wait until it is time for their next dose, then continue as usual.

 

If you accidentally give too much methylphenidate oral solution, talk to a healthcare professional or call an ambulance straight away.

Some side effects can be serious such as:

•  mood changes or mood swings or changes in personality
•  excessive teeth grinding (bruxism)

Methylphenidate oral solution should be stored safely and securely out of the reach of children and in the packaging it comes in. It needs to be stored at room temperature (less than 25oC) and doesn’t need to be kept in a fridge unless room temperature exceeds 25oC.

This medicine can be used for 30 days once opened. Any remaining oral solution should be disposed of after this time or when the expiry date on the bottle has passed (whichever is sooner). As methylphenidate is a controlled substance, please take any out-of-date oral solution to a pharmacy for disposal.

If you forget to take your medicine don’t take a double dose the next time. Just take one dose at the next normal time.

Safety Information

Like all medicines, methylphenidate can cause side effects, but not everybody gets them. While most people find that this medicine helps them, headache, feeling nervous and not being able to sleep are very common side effects, affecting more than 1 in 10 people. Please read the Patient Information Leaflet inside the methylphenidate oral solution packaging for full information on all of the possible side effects of this medicine.

Side effects are the unwanted things that can happen when you take a medicine. If any of the following happen or if you have any concerns about other possible side effects tell an adult you trust straight away. They can then talk to your doctor.

 

The main things that could affect you are:

  • Feeling or being sick, having tummy pains or having a dry mouth. These may only happen when you first start taking the medicine. It is best to take the medicine with food
  • Feeling worried or nervous
  • Feeling shaky, trembling or being unusually active
  • Feeling dizzy, or getting headaches
  • Being very depressed and unhappy or wanting to hurt yourself
  • Being very sweaty
  • Having different moods than usual, not being able to get to sleep
  • Skin rashes, bruising easily, getting out of breath or chest pains
  • The medicine can also make you feel sleepy. If you feel sleepy, it is important not to do outdoor sports like riding a horse or bike, swimming or climbing trees. You could hurt yourself and others.
  • Your heart beating faster than usual.
  • Mood changes or mood swings or changes in personality.
  • Excessive teeth grinding (bruxism).


Other common side effects include:

  • joint pain
  • dry mouth
  • high temperature (fever)
  • unusual hair loss or thinning
  • feeling unusually sleepy or drowsy
  • loss of appetite or decreased appetite
  • decreased weight
  • itching, rash or raised red itchy rashes (hives)
  • cough, sore throat or nose and throat irritation
  • high blood pressure, fast heartbeat (tachycardia)
  • feeling dizzy, movements which you cannot control, being unusually active
  • feeling aggressive, agitated, anxious, depressed, irritable and abnormal behaviour
  • stomach pain, diarrhoea, feeling sick, stomach discomfort and being sick.
 

These usually occur at the beginning of the treatment and may be reduced by taking the medicine with food.

 

If your child has side effects
If your child experiences any side effects while taking methylphenidate oral solution, tell a healthcare professional. This includes any possible side effects not listed in the Patient Information Leaflet found inside methylphenidate oral solution packaging. You can also report side effects directly to the Medicines and Healthcare products Regulatory Agency (MHRA) via the Yellow Card Scheme at https://yellowcard.mhra.gov.uk/. By reporting side effects, you can help provide more information on the safety of this medicine.

Seek advice from a healthcare professional straightaway if you notice changes in your child’s mood or any problems with their heart while on methylphenidate therapy.

 

Do not take Methylphenidate if:

    • you are allergic to methylphenidate or any of the other ingredients of this medicine (listed in Section 6)
    • you have a thyroid problem
    • you have increased pressure in your eye (glaucoma)
    • you have a tumour of your adrenal gland (phaeochromocytoma)
    • you have an eating problem when you do not feel hungry or want to eat – such as ‘anorexia nervosa’
    • you have very high blood pressure or narrowing of the blood vessels, which can cause pain in the arms and legs
    • you have ever had heart problems – such as a heart attack, uneven heartbeat, pain and discomfort in the chest, heart failure, heart disease or were
      born with a heart problem
    • you have had a problem with the blood vessels in your brain – such as a stroke, swelling and weakening of part of a blood vessel (aneurysm), narrow or blocked blood vessels, or inflammation of the blood vessels (vasculitis)
    • you have mental health problems such as:
      – a ‘psychopathic’ or ‘borderline personality’ problem
      – abnormal thoughts or visions or an illness called ‘schizophrenia’
    • signs of a severe mood problem like:
      – severe depression, where you feel very sad, worthless and hopeless
      – mania, where you feel unusually excitable, over-active, and un-inhibited.

 

Do not take Methylphenidate if any of the above apply to you. If you are not sure, talk to your doctor or pharmacist before you take Methylphenidate.

This is because methylphenidate can make these problems worse.

Check with your child’s doctor or pharmacist before giving any other medicines to your child whilst they are taking methylphenidate oral solution, including herbal and complementary medicines.

 

Do not give methylphenidate if they are taking a medicine called a ‘monoamine oxidase inhibitor’ (MAOI) used for depression, or have taken an MAOI in the last 14 days. Taking an MAOI with methylphenidate may cause a sudden increase in their blood pressure.

Share this information with your child

If you feel unwell in any way while you are taking your medicine please tell an adult you trust straight away. Other things to remember:

  • Make sure you keep your medicine in a safe place, so that no one else takes it, especially younger brothers or sisters.
  • The medicine is special for you – do not let anyone else have it. It may help you, but it could hurt someone else.
  • If you forget to take your medicine don’t take a double dose the next time. Just take one dose at the next normal time.
  • If you do take too much medicine, tell your mum, dad or carer right away.
  • It is important not to take too much medicine or you will get ill.
  • Don’t stop taking your medicine until your doctor says it’s OK.
